Summit Osteopathy is committed to providing you with the highest levels of service. This includes protecting your privacy. The Privacy Act 1988 sets out a number of principles concerning the protection of your personal information. We recommend that you keep this information for future reference, or visit this page for future reference.

 

Your personal information

Personal information held by Summit Osteopathy may include your name, date of birth, current home address, home phone number, mobile phone number, email address, Medicare number. We also are regulated by Australian Health Practitioner Registration Agency (AHPRA) that require us to keep up to date, accurate and detailed clinical notes. This is all for the express purpose of providing a you a quality health care service. It is required that we keep your records for 7 years, and if you are a minor, 7 years after + any time we have seen you before the age of 18.

 

How we collect your personal information

We collect personal information in a number of ways, including

  • Directly from you via phone, email when making initial bookings.

  • From questionnaires that you complete, e.g. intake forms, pain questionnaires

  • From you via online self-service functions on the Summit Osteopathy website (http: //www.summitosteopathy.com.au) , e.g. when making online bookings

 

How we use your personal information

Your personal information may be used to:

  • Provide a health care service to you.

  • Administer services including invoicing.

  • Inform you via email of information about the clinic, tips, or health ideas that might help you along your health journey.  

  • Communicate with you about bookings you have made, via email or text with confirmations, reminders, and changes, cancellations.

  • Gain an understanding of your information and communication needs so we can provide you with better service.

  • Maintain and develop our database

 

How we store and protect your personal information

We take reasonable steps to protect your personal information from loss, misuse, unauthorised disclosure or destruction. Your personal information is stored on a password protected database, that is stored on password or passcode protected devices, only accessible by employees of Summit Osteopathy and the Wagin Osteopathic Service.

 

When we disclose your personal information

For the purposes set out under “How we use your personal information”, we may disclose your personal information other health care providers that are part of your care team.

  • If you are accessing health care services through your GP and Medicare, we are required to give clinical reports back to your referring GP.

  • If you are accessing health care services through third parties ie. ICWA, DVA we are required to give clinical reports back to these insurers.

  • If we ever need to bring another provider into your care team, outside of the above, we will always ask for your permission to share your personal information with them, specifically relevant details of your case that will help us get the best outcome for you.

  • In the event of an outbreak of COVID-19 at the clinic, if contact tracers approach the clinic in the interest of public safety- we would disclose your name, phone number and the date and time you attended the clinic.

 

Help us ensure we hold accurate information

Summit Osteopathy takes all reasonable precautions to ensure that the personal information we collect, use and disclose is accurate, complete and up-to-date. However, the accuracy of that information depends on the information that you provide. That’s why we recommend that you:

  • Let us know if there are any errors in your personal information

  • Keep us up-to-date with changes to your personal information

 

You can access your personal information

You have the right to access your personal information, subject to some exceptions allowed by law. If you would like to do so, please contact us. You may be required to put your request in writing for security reasons. Summit Osteopathy reserves the right to charge a fee for searching and providing access to your information to recover administrative costs.

 

Privacy Statement accepted standards of technology and operational security in order to keep your personal information safe. Only authorised Summit Osteopathy clinicians can access your clinical records, and administrators within the company can only access your contact details and details required for invoicing purposes. We have policies in place to reasonably ensure your personal information is kept safe.
